Watchdog group hits TVA on nuclear plant fire safety

The Nuclear Regulatory Commission found fire protection shortcomings of "substantial safety significance" at TVA's Browns Ferry nuclear plant in North Alabama. Stephen Smith, executive director of the Southern Alliance for Clean Energy, criticized TVA's response, stating the findings represent a cultural breakdown at the plant and calling for systemic changes to prevent future safety lapses.
This should be a red flag for everybody. That shows a cultural breakdown at the plant relative to redundancy and safety.
